Enroll in 2019 Benefits Nov. 1 – 30

It’s that time of year again! Open enrollment for 2019 benefits, that is.

The enrollment period is Nov. 1 – 30. You must make final elections/changes by 11:59 p.m. EST on Nov. 30 in order for the changes to take effect Jan. 1, 2019.

The benefits guide outlines the three medical plans offered in 2019. Please see the HR Business Partner at your location for a printed copy of the guide, as well as the medical, dental and vision premium rate sheet. All of the documents are available on the ADP website (mylifecare247.com for hospital and LMS associates and https://my.adp.com for home health associates). You are also encouraged to attend an informational meeting at your location, if you haven’t already. Watch for times/dates from your HR Business Partner.  You will earn 35 Go365 points just for attending the meeting!

2019 Changes

During the associate engagement survey earlier this year, you told us the current plans are difficult to understand; that we need better choices; that we need a better co-pay plan option; and that we need an option more focused on premiums. We listened, and are pleased to offer three cost-effective, quality medical plans to you and your eligible dependents during the 2019 plan year:

  1. Co-pay Plan (deductible is $1,750 annually)
  2. HSA 2750 Plan (deductible is $2,750 annually)
  3. HSA 6500 Plan (deductible is $6,500 annually)

The Co-pay and HSA 6500 plans are brand new for 2019. No changes were made to the HSA 2750 plan for 2019.

This is considered a passive enrollment, meaning you are not required to take action unless:

  1. You want to make changes to your medical, dental or vision elections and/or
  2. You participate in the Flexible Spending Account (FSA) for health care or dependent care or the Health Savings Account (HSA) programs. The IRS requires you to submit your per pay period contribution amount, even if you want to contribute the same as the previous year.

If you do not make any changes to your medical, dental or vision elections during the enrollment window (Nov. 1 – 30), you will be automatically enrolled in the corresponding plan, effective Jan. 1, 2019.

>> If you were previously in the HSA 1750 or co-pay plan, you will move to the new co-pay plan in 2019, if you do not make any changes to your elections by 11:59 p.m. EST on Nov. 30.

>> If you were previously in the HSA 4500 plan, you will move to the new HSA 6500 plan in 2019, if you do not make any changes to your elections by 11:59 p.m. EST on Nov. 30.

>> If you were previously in the HSA 2750 plan and you do not make any changes to your elections by 11:59 p.m. EST on Nov. 30, you will stay in the HSA 2750 plan.

How to Enroll Online

Hospital and Support Center Associates – if you are using Internet Explorer on the LifeCare network, visit MyLifeCare247.com and you will be logged in automatically. If you are logging in outside of the network, or using another browser on the network, follow these steps:

>> Go to MyLifeCare247.com

>>Enter the following credentials:

User name: UserID@lms.ad (example 12345@lms.ad)
Password: Your network password

>>Click Sign In/Log In

Home Health Associates – go to https://my.adp.com.

>> Log in using previously established credentials or click on REGISTER NOW.

>>Your registration code is MyLifeCare – Hospitals.

>> Follow the prompts from there.

Important Things to Know

Wellness Incentive:

LifeCare offers a $25 per pay period discount on all three medical plans. In order to receive the discount, you must reach Bronze status in our Go365 wellness program by Dec. 15, 2018. There are three ways to reach Bronze (choose one of the following activities):

  1. Complete a health assessment
  2. Get a biometric screening
  3. Log a verified workout

Go365 is available to Full Time and Part Time associates, not including dependents. Associates enrolled in a LifeCare medical plan will be automatically enrolled in the program. Those not enrolled in a LifeCare medical plan may choose to sign up during Annual Enrollment.

Spousal Surcharge:

Hospital and Support Center associates who enroll a spouse in a LifeCare medical plan may be required to complete a spousal surcharge verification form. If an associate’s spouse is eligible for other employer-sponsored health care coverage (not including Medicare), the associate will be required to pay a $50 surcharge per pay period. Associates must complete the spousal verification form if they attest that their spouse is not eligible for other coverage. The form is due within 30 days of an associate’s benefit effective date, and by Dec. 15 each year during annual enrollment. Associates who do not complete the form on time will be assessed the surcharge.
